Sign your consent and treatment plan

Signing your paperwork is one of the most important steps in getting your child ready for care. This article explains where to sign your intake (consent) documents and your child's treatment plan in the parent portal, what each signature unlocks, and how to find anything that's still waiting on you.

Where signatures appear

Everything you need to sign shows up on your portal home page. After you log in, look at the Overview page (the first screen you land on). While your family is getting ready to start care, this page collects the items that still need your attention, including any documents that need your signature.

You don't need to hunt through menus. If a signature is required, it appears right on the Overview as a clearly labeled to-do. When there's nothing left to sign, those items simply stop appearing.

Signing your intake paperwork (consent forms)

Your intake paperwork is the set of consent and agreement documents your clinic needs on file before your child can begin. On the Overview you'll see an Intake Paperwork section that lists each document, with a badge showing how many are left (for example, "2 of 4 remaining"). Once everything is signed, the badge changes to All Signed.

To sign:

  1. Tap any document in the list to open it, review it, and add your signature.

  2. To move faster, use Sign All Documents to review and sign everything for this child in one flow.

  3. If you have more than one child in care, you'll also see Sign for All Children, which lets you handle paperwork for your children together.

Each signed document is marked with a checkmark and the date you signed it, so you always have a record of what's done.

Signing the treatment plan

After your child's assessment, their clinician creates a treatment plan (sometimes called a plan of care) that describes the goals and recommended therapy. When it's ready for you, a Treatment Plan signature needed section appears on your Overview.

Select Review & Sign to open it. A window titled Sign Treatment Plan shows the full document on one side so you can read it, and a place to sign on the other. To complete it, you'll:

  • Read your child's treatment plan (you can also open it in a new tab for a larger view).

  • Type your printed name and draw your signature.

  • Check the box confirming you are the patient's parent or caregiver and consent to the plan shown.

  • Select I Agree and Sign.

What your signature unlocks

Your signatures aren't just paperwork; they move your child's care forward.

  • Consent / intake paperwork is part of getting your child ready to begin. These documents need to be on file before care can start.

  • For some insurance plans (such as Tricare, Cigna, and ChampVA/Spina Bifida), your insurer requires your signature before the treatment plan can be submitted for approval. If your plan is one of these, you'll see a message on the portal noting that the plan "has not been submitted to your insurance and will not be submitted until you sign." Signing lets your clinic submit the plan to your insurance so ongoing therapy can be authorized. If your plan doesn't require your signature for submission, your clinic can submit the plan on your behalf, but signing is still needed to confirm you've reviewed and agreed to the plan of care.


If your insurance does require your signature before submission, signing promptly helps avoid delays in your child's therapy.

Finding pending signatures

If you're not sure whether anything is waiting on you, go to your Overview page. Any pending intake paperwork or treatment plan signature will be listed there. If you have several children, make sure you're viewing the right child; signature requests are shown for the child they belong to.
